Our Social Prescribing Team can help connect you with local services and community organisations that offer practical, non-medical support. They can provide advice and signpost you to help with:

- Employment, education and volunteering
- Healthy lifestyle support
- Support for unpaid carers
- Accessing local community services

If you need help to live independently because of illness, disability, frailty or age, Leeds Adult Social Care can assess your needs and advise on home care, equipment and adaptations, occupational therapy, carers' support and other services that may help you remain safe and independent.
