How to update your details
Please use the form below to advise us of any changes to your personal details such as a change of address, phone number, email address or surname so we can update our records.
Please be aware that for us to complete some future requests (ex. requesting medical records) you may need to verify your changes in person before they can be processed.
Alternatively, you can call us, or visit the practice in person, to inform us of changes to your personal details.
Change of address
If you have moved to a new address, you will need to check your new address is in our catchment area to stay registered with our surgery.
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your date of birth or name
You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
Please only complete the fields that need updating. There is no need to enter information that has not changed. For example, if you are only advising us of a change to your telephone number, you only need to provide your old and new telephone number; you do not need to complete your address or email details. This helps us process your request quickly and accurately.
