This page provides information about services and support that you can access directly, without needing an appointment with your GP. Where available, links and contact details are provided for self-referral.
- For information about your problem or medical condition, access information via NHS health conditions or Patient.info.
- Talking therapies can help with many different problems. You can refer yourself to NHS talking therapies for anxiety, depression for many different conditions.
- To refer yourself for counselling or therapy such as CBT or online therapy, please visit the Leeds Mental Wellbeing Service.
- To refer yourself for help with alcohol or drug addiction, please visit the Forward Leeds website or call them on 0113 887 2477.
- To find out about other local support teams please visit the Mindwell website or alternatively ask a receptionist to book you into a “Healthy Minds” consultation.
- Mental health support for men: Leeds Mind MEND and Andy’s Man Club.
- Social Prescribing is a way of linking individuals with a range of local community services to improve social, emotional and mental wellbeing. To access our Social Prescribing service for people in Leeds refer yourself via Linking Leeds or call them on 0113 336 7612.
- For support and counselling following a bereavement, refer yourself to Cruse Bereavement Support.
If a child or young person is needing help with their mental health, please consider the following resources:
- Kooth – an anonymous online counselling and wellbeing platform.
- The Market Place Leeds – available for 11-25 year olds in Leeds. They offer a range of services including one to one support, counselling, group-work and drop-in sessions.
- The Citizens Advice Bureau offers free support and advice about wide range of subjects, including Benefits, Tax Credits, debt, housing and employment. Visit Citizens Advice Leeds or call them on 0808 278 7878.
- Support for carers: Carers Leeds.
- To access local help for the homeless, visit Street Support Leeds.
- To access local help for asylum seekers, visit LASSN.
Social prescribing is a holistic approach that can help with non-medical issues that may affect your health and wellbeing. It connects you with local community services and activities that can support your individual needs, interests and goals.
Support may include activities and services such as:
- Arts and crafts groups
- Gardening or walking clubs
- Debt advice or housing support
- Volunteering opportunities
- Mental health and peer support

Stopping smoking is one of the best things you can do for your health. It reduces your risk of serious health problems, including heart disease, stroke, lung disease and cancer, and can improve your breathing and overall wellbeing.
If you would like help to stop smoking, you can refer yourself to a local stop smoking support service through the NHS stop smoking support service.
If you are concerned about your use of alcohol or drugs, support is available. Getting help can reduce the risks to your health and wellbeing and help you make positive, lasting changes.
Forward Leeds provides confidential support, advice and treatment for people who want to reduce or stop their use of alcohol or drugs. They can support people with a wide range of needs, from early intervention through to more complex treatment and recovery support.
- For support and advice to stop smoking, with weight loss (including a free 12 week course), exercise and diet visit One You Leeds.
- Support for the BAME community: BAME Health and Wellbeing Hub.
- For self-guided exercises to help with knee, ankle, foot, back or neck pain visit Versus Arthritis exercise sheets.
- For joint or back pain, book directly into a NHS physiotherapist by calling reception and asking to be referred to “Physio First”.
- Patient Tailored Care for MSK Pain and Women's Pelvic Health – digital self-management support for all common MSK (joint, muscle and bone) conditions and women's pelvic health.
- Evidence-based support is available for new, recurrent and long-term MSK injuries and conditions, including the physical, mental, societal, work, economic and behavioural components of recovery and wellbeing.

- For help with incontinence or stoma problems, complete a self-referral to the CUCS team (continence, urology and colorectal team). They can assess and prescribe products such as continence pads. Access via Leeds Community Healthcare – CUCS.
