Most appointments are provided on the same day, so please ensure you are available to attend the practice or take a phone call when submitting your request. For appointments with our nursing team (for example vaccinations, cervical screening, or wound care), please contact the practice by phone using the numbers below.
Home Visit Requests
If you are housebound and need an appointment, please submit a form using this link before 10:00am. A GP will contact you by phone to discuss your needs and assess the most appropriate care.
Home Visit Request Policy
All requests for home visits will be clinically triaged by an appropriate clinician on the day of request. Home visits are provided based on clinical need and patient circumstances, not on request alone. Requests received before 10am will normally be considered for same-day visiting where clinically indicated. Requests received after this time, or when clinical capacity has been reached, will still be assessed for urgency, but the practice may not be able to provide a same-day home visit. Where a patient is assessed as requiring urgent or same-day care that cannot be safely delayed, they will be directed to the most appropriate alternative service, including NHS 111, urgent treatment centres, or emergency services (999/A&E), with clear safety-netting advice provided.
In accordance with guidance from the Royal College of General Practitioners (RCGP), home visits are provided only for patients whose medical condition prevents them from attending the surgery. All requests for home visits are assessed by a GP and prioritised according to clinical need.
Patients are expected to attend the surgery wherever possible. The surgery is the most appropriate setting for consultations, as GPs have access to examination facilities, diagnostic equipment and support services that are not available during a home visit.
A home visit may be appropriate for patients who are:
- Housebound or bedbound
- Receiving end-of-life care
- Too unwell to travel safely to the surgery
- At risk of serious harm if moved
A home visit is not appropriate for patients who:
- Do not have access to transport
- Find it inconvenient to attend the surgery
- Are mobile and able to leave their home
- Are children or young people who can be brought to the surgery
- Would be more appropriately managed through another healthcare service
Following clinical assessment, the GP may decide that an alternative form of care is more appropriate, such as a telephone consultation, a surgery appointment, referral to another healthcare service, or attendance at an urgent care centre or hospital.
Please note that requesting a home visit does not guarantee that one will be provided. The decision to undertake a home visit, and the timescale in which it is carried out, rests solely with the GP and is based on clinical need in accordance with RCGP guidance.
A family member, friend, or carer can complete this form on your behalf. If you are unable to use the online system, please contact the practice and our care navigators will be happy to help you complete the request.

Please do not submit an online request if you or the person you are contacting us about has any of the following symptoms. Call 999 or attend A&E immediately:
